Heart Failure Program Goal:

To improve patient outcomes, enhance the quality of life, and reduce readmission through physical exercise, dietary modification, and comprehensive heart failure education. 

Guidelines for Admission:

  • Systolic or Diastolic Heart Failure
  • Recent CABG or Heart Valve Surgery
  • Presence of LifeVest or Defibrillator

Heart Failure Program:

The PAM Rehabilitation Hospital of Corpus Christi Heart Failure Program is an innovative and comprehensive program designed to improve functional capacity and quality of life through a customized exercise program, intensive monitoring including telemetry, and patient education in a structured setting. A transitional program between Phase I inpatient cardiac rehabilitation and Phasee II outpatient cardiac rehabilitation. 

Outcome Measures:

  • Improvement in the 3-6 minute walk test
  • Overall patient satisfaction
  • Functional change in distance walked
  • Daily weight monitoring
