Referral and Assessment Process

The hospital case manager or physician will meet with you before you are admitted to a Post Acute Medical rehabilitation hospital to determine if a referral is appropriate. If this does not occur, patients have the right to ask for a rehabilitation assessment to see if they meet criteria. Patients also can be referred from home, a physician’s office, assisted living facilities, skilled nursing facilities, and the emergency room. A clinical rehabilitation specialist nurse or therapist performs an assessment at no charge to the patient. It is imperative for patients and their families to know that if they meet the criteria for rehabilitation, they have the right to determine the rehabilitation center in which to receive care. This decision is crucial because it can make the difference in maximizing recovery.

Once You Arrive 

Once you have been admitted, your rehabilitation journey begins with a comprehensive physical examination by a physician. This is the first step in determining your detailed treatment plan. Throughout your stay you will be under the care of trained professionals focused on providing you with the most effective treatment possible. At the same time, we encourage you to interact with your fellow patients with similar conditions because they understand what you are experiencing and can provide invaluable support.

Preparing for Discharge

Armed with the training you received and the appropriate resources based on your individual need, you will approach discharge with the necessary confidence to manage your continued recovery. Our team of clinical rehabilitation experts will help you transition from inpatient rehabilitation to home or whatever setting is appropriate for your ongoing rehabilitation program, ensuring you and your family continue on the road to recovery.

Insurance
